Adult Services Outreach
Outreach services include delivery of MPLD materials to you in your home.
In order to qualify for MPLD Outreach services you must have a current valid MPLD library card and meet the definition of a Homebound patron.
DEFINITION OF A HOMEBOUND PATRON
1. A physician's verification of homebound status is required either for temporary or
permanent delivery of items.
2. Leaving home is a major effort.
3. You are normally unable to leave home unassisted.
4. When you leave home, it must be to get medical care, or for short, infrequent non-
medical reasons such as a trip to get a haircut, or to attend religious services or adult
day care.
TO APPLY FOR HOMEBOUND OUTREACH SERVICES
In order to be considered for Outreach services, you must fill out an eligibility questionnaire available at the Public Services Desk, or you may request one be sent to you by mail by calling the Adult Services Department at 815-385-0036.
